Refer Family and Friends
It’s a win-win situation when you refer family and friends. We reward you and we also reward them when they come in for their first appointment.
How It Works
- Use the Referral form (above) to submit your information and that of the person you are referring. You can refer as many people you like (but must complete the form per person). A confirmation email will be sent to you on submission of your referral for your records.
- Once we’ve received your referral, we will contact the person(s) to set-up their first appointment.
- After your referred person has completed their appointment, we will contact you regarding your referral success as well as to confirm and arrange delivery (or pick-up) of your referral reward(s).
- Referred persons will also receive a welcome gift on arrival. If the person referred does not come in for their appointment, we will not be able to issue your reward.
- If you are successful in referring a new patient to our practice, you will be able to choose gift card rewards which can be redeemed at select businesses in the Mission neighbourhood.
